Late

Clients have a 10 minute grace period to arrive to appointment. A late fee of $10 will be added to your final service fee. After 15 minutes, your appointment will be cancelled.

Arrival

Please arrive without any eye makeup and do not curl your lashes on the day of your appointment.

No Refunds

Deposits and services offered are non-refundable after payment. If there are any issues post service, send me an email or dm and we can resolve.

Service Refusal

You can be refused service if you have pink eye, stye or other issues that could increase infection.

Squeeze-Ins

Squeeze-in, early morning and after hour appointments will be charged with an extra $25. Contact via email or DM to schedule this type of appointment.

Mobile Services

We offer mobile lash services at your home/business site. Contact via email or DM to schedule this type of appointment.

Deposits

A non-refundable deposit of $15 is required to secure your appointment. Send your deposit to btmascara@gmail.com. Deposits should be sent within 48 hours or your appointment will be cancelled.

I understand and agree to the following:

  • Proceeding with this booking means I decline a patch test and I accept full responsibility for any reaction due to known or unknown sensitivities/allergies.

  • Brow Lamination uses a perming system and neutralizer to set brow hairs in a desired position. Results vary and cannot be guaranteed for a specific duration.

  • I must remain still throughout the procedure to avoid injury or damage.

  • Despite all precautions, irritation, redness, allergic reaction, or skin sensitivity may occur.

  • If complications occur, I will contact my technician and consult a physician at my own expense.

  • I am responsible for daily aftercare to help results last longer. Failure to follow instructions may cause results to fade sooner.

  • This service is not recommended if you have: skin conditions (eczema, psoriasis, sunburn, super-sensitive skin), recent procedures (microblading <6 weeks, lash/brow services <72 hrs, recent eye surgery/infection), use of strong medications (Accutane/Retinol, blood thinners, chemotherapy), pregnancy, or allergies or sensitivities to adhesives, solutions or tapes.

  • I confirm that I have read, understood, and had the opportunity to ask questions about this consent form.

I understand and agree to the following:

  • Proceeding with this appointment confirms that I have disclosed any known allergies or sensitivities to nail products, adhesives, gel products, or acetone. I accept full responsibility for any reaction due to known or unknown sensitivities.

  • Gel-X Nail Extensions use soft gel tips applied with professional gel adhesive and cured under a UV/LED lamp. Length, shape, and durability may vary depending on my natural nails and daily activities.

  • I understand I must remain still during the service and follow the technician's instructions to prevent injury, product mishaps, or damage to my natural nails.

  • Despite proper sanitation and professional application, possible risks include lifting, breakage, allergic reaction, skin irritation, sensitivity to products, or weakening of the natural nail if proper aftercare or maintenance is not followed.

  • I confirm that my nails are free from infections, fungus, open wounds, or contagious nail/skin conditions. If any such condition is present, the service may be refused for safety reasons.

  • If any irritation, allergic reaction, or complications occur, I will contact my technician and seek medical advice if necessary at my own expense.

  • I am responsible for proper nail care including avoiding excessive force, using cuticle oil, and following all aftercare instructions. Failure to follow instructions may result in lifting, breakage, or reduced longevity.

  • I confirm that I have read, understood, and have the opportunity to ask questions about this consent form.

  • Cashmere eyelash extensions will be applied to my natural lashes using adhesive. Results vary and cannot be guaranteed for a specific duration.

  • I must keep my eyes closed and remain still throughout the procedure until the technician instructs me to open them.

  • Despite all precautions, redness, irritation, tearing, or allergic reaction may occur. Adhesive fumes can cause stinging if eyes are opened during treatment.

  • If complications occur, I will contact my technician and consult a physician at my own expense.

  • I am responsible for following aftercare instructions to maintain the extensions. Failure to follow aftercare may cause early fallout or retention issues.

  • This service is not recommended if you have: skin conditions (eczema, psoriasis, sunburn, super-sensitive skin), recent procedures (microblading <6 weeks, lash/brow services <72 hrs, recent eye surgery/infection), use of strong medications (Accutane/Retinol, blood thinners, chemotherapy), pregnancy, or allergies or sensitivities to adhesives, solutions or tapes.

  • If complications occur, I will contact my technician and consult a physician at my own expense.

  • I agree to follow all aftercare instructions provided by my technician, including avoiding water for 24 hours. Failure to follow instructions may cause results to fade sooner.
